What are Facilitated Personal Care Plans (FPCP)?

Facilitated Personal Care Plans allow individuals to have a say in the way their personal care is executed. Using the Lanterman Act’s Bill of Rights as a guide, ACF works with individuals and their families to write comprehensive FPCPs for all levels of care. Individuals have the right to the best possible care. If the primary caregiver is compromised and/or unable to complete their usual daily care tasks, FPCPs are an essential tool for maintaining continuity of care. 

A FPCP is a set of documents that give a detailed description of an individual’s hour by hour care. Taking into account their personality, disability, physical/mental wellbeing, and safety level. The document describes personal care routines to follow, the same way, each and every time. FPCPs are written in the first person, making it clear the individual is in control over their care and life. 

FPCP Goals:

  • Helps individualize personal care
  • Prevents abuse and neglect 
  • Encourages positive behaviors 
  • Provides new staff, respite, medical, and emergency care workers, simple and clear directions so they can safely and respectfully provide intimate care. 
  • Creates continuity of care in an emergency

  • Assuring privacy while providing personal care 
  • Detailed personal care routines for eating, bathing, dressing, toileting, and more
  • Guidelines for lifting, transfers, and positioning 
  • Where care should be provided to ensure privacy

  • Asking Permission before touching – “NO means NO”
